An accredited healthcare facility and a board certified physician is assigned based on the address provided in your Appointment Request.
To determine the closest facility, you can call our appointments line or use our online facility selection map by using our Request An Appointment Form.

The facility is always located in the general metropolitan area of Springfield, OR.

 

Is there a wait to schedule an appointment at the Springfield locations?
